I have read and understand the terms and conditions set forth in the Company Tuition Assistance Policy.  As a condition of participating in this Program, I agree to abide by these terms and conditions. I understand that nothing in this policy should be construed as creating a contract of employment or a guarantee of continued employment for any specific period of time. I further understand that grades or other evidence of satisfactory completion, as well as an itemized invoice for all expenses must be provided.

I affirm that any documentation I have submitted in connection with this program is complete and accurate. I acknowledge that if I knowingly submit false, deceptive or misleading information in order to receive benefits under the Program, this may result in adverse tax consequences and discontinuance of my tuition assistance.

I understand that to comply with IRS provisions, tuition reimbursements above $5,250 will be processed by EdAssist with applicable Federal and FICA tax withholding. Tuition reimbursements that exceed $5,250 will also be reported on my W-2 for the year in which I receive the reimbursement. The $5,250 exclusion is determined on a calendar year basis (January 1 – December 31) and may impact me if I receive reimbursements over $5,250 in one calendar year.
